Medicare Advantage in Puerto Rico: A Front Desk and Billing Playbook for Health Centers
The short answer
In Puerto Rico, Medicare Advantage is not an edge case for a health center. It is a daily payer, often for the same patients who also have Vital through a dual eligible Platino plan. Getting it right takes three things: verify the exact plan, product, and PCP assignment in the plan's portal before the visit, register the patient under the right payer order in the EHR, and bill the encounter so the plan pays and any FQHC supplemental payment is claimed. Most denials and rework on Medicare Advantage visits start at the front desk, not in billing.
Why Medicare Advantage is different at a Puerto Rico health center
- The card in the wallet may be wrong. Patients keep old cards, switch plans during enrollment periods, and move between products inside the same carrier. "Active" in a clearinghouse response does not tell you the product or whether the patient is assigned to your center.
- PCP assignment lives in the plan portal. Many Medicare Advantage products in Puerto Rico require the patient to be assigned to a primary care provider. If the assignment is not your center or your provider, the visit can be denied even when coverage is active.
- Dual eligible patients carry two coverages. Platino plans combine Medicare Advantage with Vital benefits for dual eligible patients. The desk has to register both correctly so the claim goes to the right payer first.
- Payment rules for health centers are specific. When a Federally Qualified Health Center has a contract with a Medicare Advantage plan and the plan pays less than the FQHC's Medicare PPS amount, Medicare can make a supplemental payment for the difference. The rules are in the Medicare Benefit Policy Manual, Chapter 13. If the encounter is not billed and tracked correctly, that payment is never claimed.
- Enrollment periods move patients every year. The Medicare Annual Election Period runs October 15 to December 7, and the Medicare Advantage Open Enrollment Period runs January 1 to March 31. Expect plan changes to show up at the window in January.
The front desk and billing playbook, step by step
- Verify before the visit, not at checkout. For every scheduled Medicare Advantage patient, check the plan portal the day before: plan, product, effective dates, and PCP assignment.
- Compare the portal to the chart. If the insurance tab in the EHR still shows last year's plan or product, update it before the patient arrives.
- Flag PCP mismatches early. A patient assigned elsewhere needs a reassignment request or a scheduling decision before the visit, not a denial after it.
- Register dual eligible patients with both coverages. Record the Medicare Advantage or Platino plan and the Vital coverage in the right order, using your payer rules.
- Bill the encounter correctly. Make sure the visit is coded as an FQHC encounter for the plan and that your billing team tracks whether a supplemental payment applies.
- Watch January closely. Run a full eligibility pass on Medicare Advantage patients in the first weeks of the year and after March 31, when plan changes take effect.
- Measure the exceptions. Track how many Medicare Advantage visits each week had a plan change, a PCP mismatch, or a denial tied to registration. That list tells you where to fix the procedure.
Common gaps we see
- Eligibility comes back green from the clearinghouse, so nobody opens the plan portal, and the PCP mismatch shows up as a denial weeks later.
- Platino patients are registered with only one coverage.
- Plan changes from the enrollment periods are found at the window instead of the night before.
- Supplemental payment on Medicare Advantage encounters is not tracked, so nobody knows whether it was claimed.
- Each staff member checks portals a different way, so results depend on who worked the desk that day.
These are procedure and data problems inside the system most centers already run. See eligibility verification in eClinicalWorks and the FQHC check-in system post for the registration side.
Where Millennial Networks fits
Millennial Networks has supported healthcare IT for community health centers in Puerto Rico and the USVI since 2017, inside eClinicalWorks environments. We help centers align the registration and billing procedure with the system and clean the data that feeds eClinicalWorks UDS reporting.
MediBOT, the healthcare automation platform built by Millennial Networks, runs eligibility checks against payer portals in production today. Every night it reads the next day's schedule, checks the portals that answer for this market, including Assertus, IMC, MCS, PSM, and Triple-S, and writes the verified result to each chart in eClinicalWorks, so the desk starts the morning with exceptions only. See payer portal automation in eClinicalWorks. Across 3 Puerto Rico community health centers, MediBOT processed 16,990 automated transactions, redirected 1,088 staff hours, and touched 13,059 patient interactions during the measured period from December 2024 to July 2026. Additional workflows such as prior authorization, results alerts, and state portal lookups are scoped in an assessment, based on each organization's payers and volume. MediBOT does not make billing policy or clinical decisions; your team does.
For centers where Medicare Advantage denials or rework are the main concern, the usual path is a discovery call, then an RCM Revenue Leak Audit that maps where revenue is lost and picks the first workflow to fix. See also eClinicalWorks optimization for 330 centers.
Who this is for
- CEOs, CFOs, and COOs at FQHCs and community health centers in Puerto Rico with a large Medicare Advantage and Platino patient base.
- Revenue cycle and billing leads who work Medicare Advantage denials and supplemental payments.
- Front desk and patient access managers who verify coverage and PCP assignment every day.
Frequently asked questions
#### How should a Puerto Rico health center verify Medicare Advantage coverage?
Check the plan's own portal before the visit, not only a clearinghouse response. Confirm the plan, the product, the effective dates, and the PCP assignment, and update the insurance tab in the EHR if anything changed.
#### What is a Platino plan?
In Puerto Rico, Platino plans are Medicare Advantage plans for dual eligible patients that coordinate with Vital, the government health plan. The front desk has to register both coverages so the claim goes to the right payer first.
#### Why are Medicare Advantage visits denied when coverage is active?
The most common reasons are a PCP assignment to another provider or center, a product change the chart does not reflect, or a plan change during an enrollment period. All three can be caught the day before the visit.
#### Do FQHCs get paid the PPS rate for Medicare Advantage patients?
When an FQHC contracts with a Medicare Advantage plan and the plan pays less than the FQHC's Medicare PPS amount, Medicare can make a supplemental payment for the difference. The rules are in Chapter 13 of the Medicare Benefit Policy Manual. Your billing team should track it for every eligible encounter.
#### When do Medicare Advantage plan changes hit the front desk?
The Medicare Annual Election Period runs October 15 to December 7, with changes effective January 1, and the Medicare Advantage Open Enrollment Period runs January 1 to March 31. Plan an eligibility pass at the start of January and again in April.
#### Can eligibility checks for Medicare Advantage be automated?
Yes. MediBOT checks payer portals overnight for scheduled patients and writes the verified result to the chart in eClinicalWorks, so staff work only the exceptions.
