This checklist breaks it into the 57 intake controls in 11 domains from the CHAI AI Intake Playbook v1.0 (aligned with CHAI, not endorsed by it), adds 6 checks specific to health centers, and tells you which evidence to ask for by tier. What is CHAI?
The Coalition for Health AI publishes intake controls that any health system can use. We use them as the base and add health center checks.
What are the FQHC checks?
A BAA that covers the model provider, no training on your data without written permission, testing in Spanish and Spanglish, patients knowing it is AI and able to ask for a person, data deletion at exit, and two health center references you call yourself.
When should we use it?
